There are numerous ways to achieve this. The easiest
and quickest method is to follow below:

Its a two stage process:
1. Create a Record Insert Page.
2. Create a Field Capture page. This will pull the
information from either session variables or if you
wanted direct from the Database and then use your
email program (formmail) to send the details using a
detail confirmation page. When they click that the
details are ok, the form is then sent.

It all depends on the database you are using, the
format of the website pages (PHP,ASP,CF,etc).
