Can anybody share with me the sample questionnaire that you've used to analyze
learning needs and training needs?

I'm looking for questionnaire for the clients who are implementing SAP for the
first time. Modules may be: FI, HR, SD, and P2P.

Also, it will be helpful if you can share a sample 'design document' for the
same.

You may delete the client's name if you wish.
