I dont have a lot of knowledge about this topic, but I´l try to help you. I´m
work as process engineer in Brazil, working with P&ID´s, but I never saw a good
reference about P&ID creation. Most of companys have their own rules about
sysmbols to represent process equipments and about lines. For instrumentation,
the best reference to search is the ISA rules. In Petrobras for example we have
to follow ISA representantion. I would recomend a study of this two references
about instrumentation in P&ID:
ISA 5.1 - Instrumentation Sysmbols and Identification
ISA 5.3 - Graphic Symbols for Distributed Control/Shared Display
Instrumentation, Logic, and Computer Systems
For non-instrument sysmbols, I recomend, for exemple, a visit to the website of
Process Industries Practice (PIP - www.pip.org ). There is a lot of other
organizations that develop the same kind of work, as DIN, and others. You will
find a lot of info about P&ID in this places.
One of the things that I recommend for you is to download, from Autodesk
(www.autodesk.com), an evaluate version (30 days) of AutoCAD P&ID. This tool is
a customized AutoCAD for P&ID work. This realy enables a lot of shortcuts on
P&ID "authoring", reducing the time wasted with drawing. Try to complete the
tutorial to better understand, not only the usage of this tool, but the P&ID
creation job. One of the most important things in this type of tool is the
ability to manage info in P&ID in conection with a database. That´s all when we
are talking about a "line list", for example, a document that we use to
reference the "job" of every line in the P&ID and to help the teams working with
pipe modeling.
But P&ID isn´t only a tool for knowledge transmission during the project of a
plant. It will be used during the entire life cycle of a plant, helping
maintenance teams, helping future works on change the capacities, helping safety
assessment teams during the HAZOP´s, etc. This is a very important documentation
not only during design but during the entire life cycle of a plant.
There´s a lot of tools that are used to draw and manage P&IDwork. Try to search
for this tools, for example:

SmartPlant P&ID
AVEVA P&ID
CAD Worx P&ID
