The procedure you describe will work in Cindex if you *print* the file.
What I do, if the client wants "Index" at the top of the first page in the
RTF file, is to insert an entry
Main: <Index>
Page: {0}
The < > will make it sort to the top of the file. The {0} will keep Cindex
from beeping at me that I'm missing a page number, but suppress the comma it
would otherwise insert after "Index"

Subject: [cindexusers] adding a header to an index
I'm creating my first index exercises for a class. With SKY, a header of
"Index" was automatically created when I printed the index. With Cindex,
I've followed the instructions in the help file to add a header "%Index"
under Document, Headers and Footers, then resaved my file, then re-saved as
RTF -- but nothing happens. No header is created in the RTF file. Of course
I could manually add it in Word, but I want to know how to do it in Cindex.
What is the secret to this?
