I prefer the title Download Manager Options - Mail Setup Tab.
As for the menucascade, Generally I would just state something like:
Click <menucascade><uicontrol>menu
option</uicontrol><uicontrol>submenu
option1</uicontrol><uicontrol>sub-submenu
option1</uicontrol></menucascade> to do whatever...
Basically this interprets as follow the path to get to the lowest
option and click to complete the action. I would consider taking the
same approach through other parts of an interface, though an argument
can be made for breaking down the path into more discrete groups as
separate steps.
