The nesting of maps shouldn't affect production of PDF (or anything
else) since the toolkit resolves nested maps to a single map before
passing it to the final process.

According to the DITA 1.1 language spec, topicrefs with no explicit format attribute inherit the setting from the nearest ancestor that has the attribute set....

... The error I posted in my previous mail was caused by setting the top-level map format attribute to "ditamap" -- this used to have no effect and it appears...
