I have had some measure of success with fotonotes today. I managed to
get it working on both my testing and live sites.

However, it definitely doesn't work with mod_rewrite. I am not sure
which program would actually need to be fixed to solve it, but it's
not a big deal, because I don't mind the non-mod-rewrite URLs.

I also had to modify the URL in the .js file to:
var fnServerPath = "./fnclient/";

which seemed to resolve the pathing issues.

My image.php file in my current theme looks like:

Hope this helps someone else, and keep trying to use my files I sent
to maybe get the mod_rewrite working. Or if you have any suggestions,

I'm just glad to get it working. It's a great addition to my gallery.
