Vertical is a separate setting which is visual only.
Mail Directory defines a default mail directory that you can then use =
to to expand (much like in mutt) in the Path for each folder. For
Courier IMAP, you probably don't need to use the Mail Directory setting.

I've never used gbuffy against a courier server, but remember that the
count that gbuffy lists is fetched by doing an EXAMINE against the
mailbox, and then looking for the UNSEEN count. If you compile gbuffy
with debugging enabled, you can then start it with 'gbuffy -d 4' and it
should create a ~/.gbuffydebug file which should have information on
what is going on.
