At least one trapped worker confirmed dead in Texas manhole accident

FAIRVIEW - Authorities have confirmed that at least one of two people
trapped in a manhole is dead.


The fire chief in Fairview reported that rescue crews were able to recover
one body and the second person is missing after being swept away in the
pipeline. The workers became trapped in the multi-level hole shortly after 8
a.m. in the Thompson Springs neighborhood in Fairview. Police said a third
worker notified authorities about the situation. The workers were on a
contract job for the North Texas Municipal Water District when the incident
occurred. They were working to find a clog in the waste water line and were
on the system's last manhole when at least one of them became overcome by
either a lack of oxygen or chemical fumes. The fire chief said they believe
the first worker went into the manhole, and when he became overcome, the
second worker went in to help and also became overcome.

Worker Coverall Caught in Rotary Table

While running casing, the driller engaged the rotary clutch instead of the
hoisting clutch.

The rotary table began to turn and the slips caught the Tong Hands coveralls.
The worker

was knocked to the floor and was pulled around the drill floor by the rotary
table.
The worker suffered pulled ligaments to his right knee and was unable to return
to work the

following shift.

In addition to changing your smoke alarm batteries this weekend, the USFA
recommends

following these simple steps to protect your life, your loved ones, and your

* Dust or vacuum smoke alarms when you change the batteries.
* Test alarms once a month using the test button.
* Replace the entire alarm if it's more than 10 years old or doesn't work
properly when

want to ship as an excepted material, reclassified as combustible with no other
hazards preesent. Can they ship drums under this exception in 173.150? I am
unclear after reading it if "b" must also apply.

And once they do that, they are not subject to 49 CFR or HMR, but only within
the U.S. If they ship overseas, it's still a Flammable Liquid.

They MIGHT want to include info on their BOL that includes the flashpoint and
refer to 173.150(f), so that a roadside inspector doesn't try to put it "back
into the book."

Questions for folks shipping in the USA under 49 CFR.

A January 19, 2011 final rule changed the design of the LQ marking from a
diamond with the UN # inside the diamond to a diamond with blackened top and
bottom corners.

In your opinion, what are the transition deades for the scenarios listed below?
All questions are for shipping domestically under 49 CFR by ground, rail or
water (not IMDG or IATA).

1. What is the mandatory deadline that the old style LQ marking (diamond with
UN# inside the diamond) is no longer acceptable for packages shipped under the
LQ exception for the following situations.

a) Using packaging pre-printed with the old style of LQ marking.

b) Using packaging where the old style LQ marking was applied on the packaging
as a sticker.

c) Shipping or re-shipping filled packages where the packaging bears the old
style LQ marking (either pre-printed or applied as a sticker on the packaging)?

2. Is it prohibited to ship packages under the LQ exception if the packaging
bears the old style LQ marking in addition to the new style LQ marking or the
UN# and proper shipping name?

Hi all,

I'm looking at our medical surveillance program and trying to get a handle on
what exactly needs to be included as part of the annual exam.  Our employees are
tasked with emergency response to many different types of hazardous materials,
so their potential exposures could be quite varied.  They may work in any of a
number of western states, including some under Fed OSHA and some under state
plans.

What are other companies doing as far as medical surveillance?  I've looked at
some different recommendations and am wondering what is required / best
practice.  It seems some of the tests are at the physician's discretion, and
I've never yet had any physician tell one of our employees that they didn't need
the full package of testing / exam / bloodwork / etc.

If anyone would be willing to share what their company includes as part of their
medical surveillance program, or if anyone in a similar situation has some
advice, please let me know.  There has been some interest in cutting back on the
amount of medical testing we do, but I would like to make sure that we are
including the things we should, so any documentation or sources of information
would also be very welcome.

California Man Sentenced to Five Years in Prison for Illegally Storing Toxic and
Explosive Hazardous Waste in His Backyard

WASHINGTON – Edward Wyman, 64, from Reseda, Calif. has been sentenced to five
years in federal prison for illegally storing toxic and explosive hazardous
wastes in his backyard, materials that posed an imminent danger to nearby
residents. The sentence is the longest handed down by a California federal judge
in a hazardous waste case.

Wyman was convicted of the felony environmental crime on April 5, 2011, by a
federal jury following a five-day trial. In addition to convicting Wyman of
violating the federal Resource Conservation and Recovery Act (RCRA), the jury
made a special finding that the defendant's conduct knowingly placed another
person in imminent danger of death or serious bodily injury.

Wyman was also ordered to pay $800,000 to the United States Environmental
Protection Agency's Region 9 Emergency Response Office for costs associated with
a 47-day clean-up. During the clean-up, EPA contractors were forced to call out
the Los Angeles Police Department Bomb and Arson Squad seven times to deal with
possible explosives mixed into the burned debris.

Wyman was charged in June 2009, after firefighters responded to a report of a
fire and explosions at Wyman's residence. Because of the ammunition that was
being "cooked off" in the fire, firefighters had to wear bullet proof vests.
Investigators at the scene discovered a large cache of toxic materials,
including thousands of rounds of corroded ammunition, highly reactive
lead-contaminated waste from shooting ranges, hundreds of pounds of decades-old
gunpowder and military M6 cannon powder, and industrial solvents that contained
1,1,1‑trichloroethane and tetrachloroethylene, two potent chemicals that
are listed as hazardous substances under federal law. Wyman did not have a
permit to store any of the materials.

Marking & labeling of overpacks is covered under 49CFR173.25(a).  In
short, you must reproduce all marks/labels on the overpack unless they
are clearly visible on the inner containers.  You must also mark the
overpack with the word "OVERPACK" & add orientation labels when
applicable.

It was shock and grief today in Kalagi trading center in Kyampisi
Sub-County, Mukono district when the residents found a fuel pump attendant
dead in the underground fuel tank. The deceased has been identified as
Blavio Otheino who is reported to have died early in the morning. Othiono is
reported to have been on the night duty and that by midnight he was seen
serving customers. The police suspect that Othieno could have wanted to
siphon fuel but he choked in the process.

2 sisters of 2,5 and 5 years old, who were mutilated by an unknown suspect
last Sunday, indicated the culprit themselves: the new girlfriend of their
father, a Brasilian model.



Last Sunday morning The 37 year old consultant Axel S from Kontich drove to
the house of his ex-wife in Lint, where the 2 children live. The father was
going to take his daughters to the animal farm for a day. He then drove to
his apartment in Kontich, where he picked up his Brasilian girlfriend. The 4
were about to drive away, when Axel halted at the front door to pick some
documents from the mailbox. When he got back in his car 2 minutes later,
both children were crying. Only after a few kilometres the father realised
that it was not just some whining. When he checked the children from close
by, he only noticed that the cloths of the youngest firl were wet. He drove
the children to the Stuivenberg hospital in Antwerp. Doctors found 2nd an
3rd degree burns on breast and belly of the youngest girl, who was
critically injured. The older girl had lighter burns on her body. Surgeons
warned the police. Both the father and his Brasilian girlfriend were
arrested last Sunday.



Axel S. got to know his girlfriend in Brasil, that he regularly visits for
business. Daniela M (34), a model, is said to belong to the upper class of
her country. Axel S fell in love, left his wife and moved with his new
girlfriend to Kontich about a year ago.

The wife and the girls did not match well, according to a friend of the
family. The girls often complained to her mother about their new steph
mother. According to neighbours there were often fierce quarrels between
both adults when the children were there. Police detectives are convinced by
now that the Brasilian woman has mutilated the children. Both children
indicated her as the culprit. The father was released on Monday. There seems
to be only one motive: the steph mother wanted to get rid of the children.

Investigation of the clothes of the children made clear that a drain cleaner
was poured on them. The woman had probably put the product in a small
sprayer. The sprayer itself was not found back, despite a search in the
apartment and the car. The investigators assume that the woman threw the
bottle out of the car when no one looked.



Since last Sunday, Daniela M is in prison. She states to be innocent. She
denies everything, according to her lawyer Omar Souidi. There are not many
statements available. That is realised too by Axel S. His laqyer says that
it is hard to believe, but he realises that everything points into that
direction. There were tensions, but there were no indications that something
like this was going to happen. Axel S is now mainly occupied with the
effects on his children. Especially the youngest has been hit hard. She has
already undergone an operation by a plastic surgeon twice and is out of
danger. The eldest girl has left the hospital..

presentations for the 22nd WCDM taking place June 25th – 27th, 2012
in  Toronto, Canada at the Metro Toronto Convention Centre.

The 2012 Conference theme is:

Global Threats....Local Consequences

We will be looking for abstracts that identify global threats (eg –
natural disasters, terrorism, cyber attacks, economic meltdown, etc),
look at the impact these events have at the local level and then advise
our delegates in terms of implementing solutions or developing
strategies to mitigate the effects. We are especially looking for
immediate 'take-aways' for our practitioner audience. These can be in
the form of case studies, proven results or peer validated research and
studies

We will also be looking for abstracts that deal with the impact of the
global economic downturn in terms of reduced budgets and support for all
aspects of disaster/emergency/crisis planning (mitigation,
preparedness, response and recovery) and how practitioners can do more
with less or how they can meet their performance objectives differently
i.e. thinking out of the box. Success stories of expanded programs and
budgets in the face of this adversity are also welcome.

The program selection committee will give preference to submissions that
comply with the Theme and Objectives of the 2012 WCDM.

Something similar happened a few years ago at a refinery in Nigeria. A worker
disappeared.
About a month later, someone noticed a strange smell coming from a storage tank.
The
missing person was found inside, surrounded by half a dozen fuel cans. Not sure
whether it
affected the octane rating.

Position:  Manager Environmental Manager systems and Quality
Location:  Eastern Nebraska

Job Description:  This is more of a strategic position with several direct
reports.  The position reports to the Director of EMS and Quality.  This person
will be responsible for all aspects of QMS and EMS including, but not limited
to, directing, planning, implementing, reviewing and reporting of the management
systems.  Direct and coordinate training to all employees on all aspects related
to the QMS/EMS including but not limited to training of the standards,
procedures, etc. Keeping up to date with all standards and implementing
necessary changes. Coordinate and direct external 3rd party
registration/surveillance audits. Administration of Document Control including
but not limited to hardware/software for proper creation/modification,
communication, and review of internal/external documents. Supervise QMS & EMS
staff.  Administration of the internal audit program including developing
internal audit plan, coordinate training and development of internal auditors,
performing audits to plan, and on-going communication/reporting of results.

Qualifications:  The ideal candidate must have experience in ISO 9000, 14000 and
TS 16949 from a manufacturing operation, internal or external auditing

Location: Eastern FL approx. 100 miles North of Ft Lauderdale and NW AR

Job Description:  Develop, implement and maintain comprehensive HSSE management
process including Operational audits, corrective action tracking, MOC, Training
and incident investigation.  Assist in the preparation of HSSE procedures and
participate in HSSE training for plant personnel and provide ongoing HSSE and
PSM support for the

facility once operational.  Works closely with plant personnel to identify risk
and develop and implement control methodology. Provides review of Operations,
Maintenance, and Laboratory Standard Operating Procedures for Safety, PSM and
Environmental compliance.  Perform audits/inspections/ environmental
surveillance and use findings to target coaching, communications and/or training
to achieve consistent application,

performance improvements, and regulatory compliance.  Audits and monitors
permits including hot work, confined space entries and lockout/tagout permits. 
Meet HSSE objectives and comply with all relevant laws and policies.

Qualifications:  The person will have 5+ years of ehs experience from a chemical
manufacturing facility or similar industry including Specific experience should
include environmental permitting and PSM.  BS in Safety, Environmental Science

Job Description:  Assists supervision and management with Explosive Safety
concerns, to include Process Safety Management (PSM), Explosive Site Planning
Contractor Owned – Contractor Operated (COCO) and Government Owned-Contractor
Operated (GOCO) facilities.  Provide interpretation and guidance concerning the
application of explosive safety regulations, policies, procedures, and
standards.  Responsible for conducting and documenting illness and injury
prevention, risk assessment and mitigation, incident investigations, hazardous
materials communication and awareness, as well as safety and environmental
programs compliance.  Conduct explosives safety site plan documentation packages
for submission to the Administration Contracting Officer (ACO) and Procuring
Contracting Officer (PCO), for final approval in accordance with DOD 4145.26-M 
Participate in Process Hazard Analysis and be able to work with a team to
mitigate any hazards associated with the explosives process.  Must be able to
perform root cause analyses and corrective action on explosive safety
disciplines, where applicable.

Qualifications:  Must possess excellent communication skills, have a generalist
safety background in a manufacturing environment and have experience in
Explosive Safety programs. Full understanding of all elements of the Process
Safety management standard.  Experience with communicating explosives safety
policy to higher level leadership.  Overall generalist in EHS disciplines. 
Experience in implementing and sustaining an Explosive Safety program  in a
large manufacturing environment.  BS in Safety or related and personnel
management experience and 10+ years ehs experience in manufacturing.
