The PDF file available via the link at the bottom of

contains a note that says:

"You cannot save data typed into this form. Please print your completed
form if you would like a copy for your records".

I want to submit the form electronically rather than by snail mail, so
why on earth is saving disabled?

I tried to print the the file to PDF, but it doesn't work, even after
removing the security via Advanced PDF Password Recovery.

Any comments/suggestions?
