The easiest and best way to fix an infected Windows installation is from outside Windows--otherwise, it's like trying to do a valve job on an engine while it's running. Much of the current malware hides itself from Windows as it cripples any attempt to remove it--but it can't hide if Windows ISN'T running. That's where booting from a "live" CD is useful.
