You need to use the "Vector->Set Scale..." option to tell WinTopo
about the original scale of your drawing.
At an original scale of 1"=1' this is a scale factor of 1:12 (1 inch
= 12 inches). So, in the box for "Scale of original image 1:" you
should enter the value 12.
You report that the drawing is coming out at 46 inches (3'10")
instead of 276 inches (23 feet). This is a scaling factor of 1:6.
So, you should check that the "Raster resolution" box is showing the
correct scanning DPI. WinTopo can only get the right DPI if it is
included in the raster file. Sometimes raster files do not include
their own DPI information, and in this case WinTopo just uses the
DPI of the previous raster, as a guess. The initial default for
WinTopo is 100 DPI, and so with the 12:6 discrepancy it would
suggest that your raster image may have been scanned at 200 DPI.
Finally, check that the "Units for vector drawing" option is
selected appropriately for your system. We would guess that "inch"
would be your selection. (Users outside of the US will probably want
to select "m" or "mm" for metres or millimetres).
After doing the "Vector->Set Scale...", you can then save the vector
drawing.
The scale settings are remembered between sessions, so you won't
need to do the "Vector->Set Scale..." operation again until you work
on a drawing with a different original scale, or if your raster
images do not contain their own DPI information and you are scanning
at a different resolution.
