The YUI extensions can be found here:

Once you have downloaded the extensions, you will need to install them
via the CS4 Extension Manager, available at:
http://www.adobe.com/exchange/em_download/. Select "install
extension", which will present a filesystem dialog. Then, select the
downloaded .mxp file to install the extension.

After launching Dreamweaver CS4, select "Insert > YUI" from the
application menubar to insert the desired YUI widget.
